Traditional medical records are organized around diseases. These domains are separated for assessment, not because they are independent. Hearing loss can increase social withdrawal. Low mood can reduce activity and appetite. Weakness can limit community participation, which may further affect cognition and wellbeing.
How Is Intrinsic Capacity Different From Disease Count?
Two people with the same diagnoses can function very differently. One may have well-managed chronic conditions, strong social support, accessible housing, and good mobility. The other may face untreated hearing loss, medication burden, isolation, and a home that makes movement difficult.
Counting diseases misses that difference. Intrinsic-capacity assessment asks what the person can do now, what has changed, and which domain may be recoverable or supportable.
This does not replace diagnosis. New cognitive symptoms, weight loss, falls, low mood, or sensory changes still require clinical evaluation. The framework helps organize care around outcomes that matter to daily life.
What Does Screening Look Like?
The WHO’s Integrated Care for Older People approach provides tools for identifying declines and guiding person-centered assessment. Depending on the setting, screening may include questions or simple tasks involving memory, chair rises, walking, weight or appetite, vision, hearing, and mood.
A positive screen is not a final diagnosis. It signals the need to look closer. Slow chair rises might reflect weakness, arthritis, pain, neurological disease, poor balance, or fear of falling. Reduced appetite might arise from dental problems, medication, depression, food insecurity, or illness.
The useful unit is not the checkbox; it is the follow-through. Assessment should lead to a plan that fits the person’s priorities, risks, and environment.
Can Capacity Improve Later in Life?
Some age-related changes are not reversible, but capacity is not a one-way staircase. Strength and balance can improve with appropriate training. Hearing aids or vision treatment may restore access to conversation and activity. Nutrition support, medication review, rehabilitation, sleep care, and treatment of depression may improve multiple domains at once.
Environmental changes can also protect function without changing the body directly. Better lighting, handrails, transport, social connection, and simplified medication routines reduce the amount of capacity required for daily tasks.
This is a central insight of the framework: healthy aging is produced by the relationship between the person and the world around them.
How Can You Use the Idea Without Medicalizing Daily Life?
Watch for meaningful change rather than chasing a perfect score. Is walking becoming slower? Are conversations harder to follow? Has appetite dropped? Are familiar tasks taking more effort? Is a person withdrawing from activities they value?
Bring those observations to routine care, especially when several changes appear together. Ask whether hearing, vision, mood, nutrition, strength, cognition, medications, and the home environment have all been considered.
For personal planning, choose actions that support several domains: resistance and balance exercise, adequate nutrition, sensory care, social activity, cognitively engaging tasks, and environments that make movement safe. None guarantees longevity, but each can preserve the abilities through which longer life is actually experienced.
Intrinsic capacity reframes aging from a list of deficits to a map of usable reserve. Its value is not in labeling a person as high or low. It is in finding the next practical opportunity to protect function.
